Lung cancer CARCINOMA PULMONAR

Reason for visiting

65-year-old male patient, smoker (until 3/02/11) with ischemic heart disease and pulmonary pathology on his medical history.

He went to the doctor because he had pain on the right hemithorax when coughing which spreads to the upper right limb what limits his mobility.

After several studies a tumour injury on the right upper lobe is detected and after making a PET it does not show a spread disease.

A surgery is made doing a resection on the upper right limb, rib, adjacent lung nodule and hilar nodules.

At this point, he shows a chest wall with adenocarcinoma infiltrating the pariental pleura and the soft part tissues, with surgical margin occupied by tumour. It does not infiltrate mediastinal adenopathies.

Therefore, we find a high risk patient, with an important risk relapse at local level after the operation and whose adenocarcinoma is on a pT4N0M0 stage.

Complementary diagnostic tests:

A new computed axial tomography of the thorax should be made, where the size and localization of the tumour and/or the metastasis can be visualized1 .

Analytical tests, where we will search for tumour markers such as carcinoembryonic antigen (CEA) and CA 19.9.

Abdominal CT and bone scan, eventually. A radiotracer that focuses on the harmed bone is used, and it could indicate the presence of bone metastasis.

Study of the exon 19, of the EFGR, we search for the existence of mutation. The objective is to see if it has mutation, as we would have a specific treatment if a metastasis appears. Inhibitors of the thymidine kinase are used on this treatment. Probably, the patient will not have the mutation, since it appears in women, in non-smokers and in Asian people. Although our patient does not fulfil this profile, he could possibly have it, so it is important to do the study to be able to be prepared in case the metastasis appears2 .

Oncological treatment for lung cancer in stage pT4N0M0:

This lung cancer of non-small cells in stage III is a cancer which has spread locally. It has already been operated, and although we treat it, it has a high risk of reappearing, in the same place or at a distant location. The treatment will consist of:

Local postoperative radiation therapy, but without including the mediastinum insomuch as there are not lymph nodes. The Radiation Oncologist administered 50 Gy.

Program of adjuvant chemotherapy. By using ectoposide and cisplatin3 .The chemotherapy could be dispense before the radiotherapy (neo-adjuvant therapy) or simultaneously (concomitant therapy)4 .

Motivo de Consulta

Paciente varón de 65 años, fumador (hasta el 3-2-11) con antecedentes de patología isquémica y pulmonar.

Acudió presentando dolor al toser en hemitórax derecho que se irradia a miembro superior derecho y llega a limitarle la motilidad.

Tras varios estudios se detecta una lesión tumoral en LSD y que tras realizar PET no muestra enfermedad extendida.

Se realiza cirugía, haciendo resección de LSD, costilla, nódulo pulmonar adyacente y nodulos hiliares.

En este momento, muestra pared torácica con ADC que infiltra pleura parietal y tejidos de partes blandas, con margen quirúrgico ocupado por tumor. No infiltra adenopatías mediastinicas.

De forma que nos encontramos con un paciente de riesgo, con recidiva importante a nivel local tras la operación y cuyo adenocarcinoma tiene un estadio pT4N0M0.

Pruebas diagnosticas complementarias:

Se debe realizar un nuevo TAC de Tórax. Donde podemos visualizar el tamaño y localización del tumor y / o metástasis 1 .

Pruebas analíticas, en las que buscaremos marcadores tumorales como el CEA y el CA 19.9.

TAC abdominal y Gammagrafía ósea, en ocasiones. Se utiliza un marcador radiactivo que se concentra en el hueso dañado y que puede indicar la presencia de metástasis óseas.

Estudio del exón 19, del EGFR, buscaremos si existe mutación. El objetivo es ver si tiene esta mutación, ya que tendremos un tratamiento específico en el caso de que aparezcan metástasis. Consiste en el uso de inhibidores de la timidin kinasa. Probablemente no la tenga, pues esta mutación aparece en mujeres, personas no fumadoras y de raza asiática. Aunque nuestro paciente no cumpla este perfil, cabe la posibilidad, de forma qu  es importante hacer el estudio para poder estar preparados en el caso de aparición de metástasis 2 .

Tratamiento oncológico de ca. Pulmonar estadío pT4N0M0:

Este cáncer de pulmón de células no pequeñas en estadio III es un cáncer que se ha diseminado localmente. Ya ha sido operado y, aunque lo tratemos, tiene alto riesgo de reaparecer, ya sea en el mismo lugar o en una localización distante. El tratamiento consistiría en:

Radioterapia locorregional  postoperatoria, pero sin incluir el mediastino puesto que no hay ganglios. El Oncólogo Radioterápico administró 50 Gy.

Programa de quimioterapia adyuvante. Mediante el uso de Etopósido y Cisplatino3. La quimioterapia puede ser administrada antes que la radioterapia (terapia neoadyuvante) o simultáneamente (terapia concomitante) 4.
