Cáncer de pulmón de células pequeñas, enfermedad extendida (CPCP-EE). Extensive Stage - Small cell lung cancer (ES-SCLC-)

Motivo de consulta:

Mujer, 59 años. Tos y fiebre.

Antecedentes personales y familiares:

Hermana con cáncer de mama derecha.

Pruebas complementarias

  • Rx y TC torácico: neoplasia pulmonar con múltiples nódulos.
  • Broncoscopia: Ca. microcítico de pulmón.

Tratamiento

Seis ciclos de cisplatino-Etopósido (PE), obteniendo una respuesta parcial y mala tolerancia con varios ingresos por neutropenia. Posteriormente, se deriva a Stereotactic Body Radiotherapy (SBRT).

El CPCP-EE es la enfermedad a distancia o que no puede ser incluida en un campo de radioterapia sin excesiva toxicidad, con muy mal pronóstico.1 El grupo VALSG realiza una clasificación del CPCP que condiciona el tratamiento, asítenemos: CPCP-enfermedad limitada y CPCP-enfermedad extendida. El CPCP es un tumor altamente agresivo y usualmente se presenta con metástasis al diagnóstico y la cirugía no estáindicada. Seis ciclos de cisplatino-etopósido (PE), obteniendo una respuesta parcial y mala tolerancia con varios ingresos por neutropenia. Posteriormente se deriva a Stereotactic Body Radiotherapy.

 

El tratamiento de inducción indicado es la quimioterapia sistémica inicial. El estándar desde hace 20 años sigue siendo la combinación de dos quimiotóxicos, usando de base el platino: cisplatino-etopósido (PE). Una alternativa y con menor toxicidad medular es la combinación de carboplatino-etopósido (CE). Se han estudiado otros tipos de tratamientos (camptotecina, irinotecan, topotecan…) sin resultados significativos. Ensayos clínicos han demostrado que las combinaciones de 3-4 fármacos aumentan la supervivencia pero presentan mayor toxicidad. 2La radioterapia torácica ha demostrado una mejora en la supervivencia en aquellos pacientes con una respuesta favorable a la quimioterapia, con evidencia de enfermedad residual en tórax demostrada con TC. La radioterapia craneal profiláctica suele indicarse también en estos pacientes, aunque el aumento de la supervivencia es dudoso.2 El futuro en el tratamiento del CPCP debe de orientarse en un mejor conocimiento de la biología de esta neoplasia y buscando nuevas dianas terapéuticas. Los días del 22 al 24 de abril del 2015 ha tenido lugar en Nueva York una reunión con más de cien expertos de todo el mundo en CPCP. Entre los ocho temas que se discutieron, destacar las “nuevas dianas y nuevos fármacos para CPCP”3El pasado 20 de Abril, Fred R. Hirsch hace una publicación esperanzadora en la AISLC: se ha demostrado la presencia de un receptor conocido en el cáncer de pulmón de células no pequeñas (FGFR-1) en casi el 23% de los CPCPs. Implica que los tratamientos dirigidos a inhibir dicho receptor, pueden ser aplicables para este subgrupo de pacientes con CPCP.4

Treatment:

Six cycles of cisplatin plus etoposide (PE) are given, getting a partial response and poor tolerance with multiple hospital admissions for neutropenia. Afterward she’s derived to Stereotactic Body Radiotherapy.

SCLC is highly aggressive tumor and usually it is presented with metastases at diagnosis when surgery is rarely indicated.

VALSG’s group makes a classification of SCLC which conditions the treatment: SCLC with limited stage and SCLC with extensive stage.

SCLC-ES is the distant disease or disease that can´t be included in a radiotherapy field without overmuch toxicity, it’s a disease with very poor prognosis. 1

The indicated induction treatment is the initial systemic chemotherapy. The Standard since twenty years is the combination of two chemotoxic drug with a platinum-based regimen: cisplatin plus etoposide (PE). An alternative to this treatment with lees myelotoxicity is a combination of carboplatin plus etoposide (CE). Another types of chemotoxic drug have been studied (camptothecin, irinotecan, topotecan…) without significant results. Many clinical trials have shown that three or four drug combinations improve overall survival but nevertheless ir increases toxicity. 2

Thoracic radiotherapy is associated with improved overall survival in patient with ES-SCLC who have had a favorable response to their initial chemotherapy and who have residual disease limited to the chest shown by CT. The prophylactic cranial irradiation may have roles in this patients but the improved overall survival is doubtful. 2

The future in SCLC treatment should aim at a better understanding of the SCLC’s biology and searching for new therapeutic targets.

In the last 22-24 April, there was a meeting in New York with more than one hundred expert in SCLS from around the world. Between the eight topics discussed, should be noted “New targets and new drugs for SCLC”. 3

Last April, 20, Fred R. Hirsch made a hopeful publication in the AISLC: it has been shown the presence of a known receptor in non-small cell lung cancer (FGFR-1) in nearly 23% of SCLC. This implies that treatment aimed at inhibiting this receptor, may be applicable for this subgroup of patients with SCLC. 4
