Breast cancer Carcinoma de Mama

Reason for visiting

A female 59 years consulting for diagnostic study and nipple retraction discovered in December 2012

Personal and Family History

  • NAMC
  • A sister with Right Breast Carcinoma

Complementary tests

  • Mammography: Breast BIRADS 4 lesion1
  • Biopsy: Infiltrating Lobular Carcinoma. Positive hormone receptors, HER2 negative
  • RM: 24x18x19 mm lesion retroareolar right BIRADS 5, adjacent another one of 56x24x25 mm
  • Thoraco- abdominal CT : nonspecific small nodular lesions
  • Negative GGO
  • Sentinel node biopsy : negative
  • Histology (after surgery): Infiltrating Lobular Carcinoma 0.7cm, free edges and positive hormone receptors

Treatment

  • Neoadjuvant chemotherapy
  • Conservative surgery
  • Hormonal treatment 5 years Anastrozole

The sequence begins to discover therapeutic nipple retraction, so mammography is done because it is the screening test for breast cancer (although according to the WHO should have received clinical examination every 2-3 years from age 20 and every year starting at age 40, and annual or biannual 50-69 from the mammogram, because her sister your risk is 1 degree Family RR 1.8)

After that the injury found to be classified as BIRADS 4 (Breast Imaging Reporting and Data System), means that it is an injury that will require intervention, but have a range of probability of malignancy broad (2-95 %), so biopsy should be performed

So Infiltrating Lobular Carcinoma is discovered as a result2

By knowing the type of injury an MRI should be performed to determine the staging of breast cancer, and there are two injuries, one BIRADS 53

Finally there is to know whether the cancer has spread, so CT, Sentinel node biopsy and GGO, which indicate that there is no metastasis are performed3

It was decided to give acting as neoadjuvant chemotherapy to see if the tumor is sensitive to it and decrease the size of the lesion in order to make conservative surgery, because if not , it should be plain radical mastectomy which is more traumatic for the patient4

Finally Hormonal treatment is given 5 years of Anastrozole (aromatase inhibitor) as it has shown the benefit because it reduces the risk of relapse and increases survival. Other options are similar LHRH analogs estrogen receptors, progesterone, oophorectomy

You can also perform postoperative radiotherapy because it reduces to one third the risk of local relapse with high perioperative rate brachytherapy, external radiotherapy technique “forward” or both together

Pruebas Complementarias

  • Mamografía: lesión en mama BIRADS 41
  • Biopsia: Ca. Lobulillar infiltrante, Receptores hormonales positivos, HER2 negativo
  • RM: lesión de 24x18x19 mm retroareolar derecha BIRADS 5, adyacente otra de 56x24x25 mm
  • TAC toraco-abdominal: pequeñas lesiones nodulares inespecíficas.
  • GGO negativas
  • Biopsia Ganglio centinela: negativo
  • AP (después de cirugía): Ca. Lobulillar infiltrante 0.7cm, bordes libres y receptores hormonales positivos

Tratamiento

  • Quimioterapia neoadyuvante
  • Cirugía conservadora
  • Tratamiento Hormonal 5 años: Anastrozol

La secuencia terapéutica comienza al descubrir la retracción del pezón, por lo que se hace la mamografía ya que es la prueba de cribado de cáncer de mama (aunque Según la OMS debería haber recibido exploración clínica cada 2-3 años a partir de los 20 años y cada año a partir de los 40, así como mamografía anual o bianual a partir de los 50-69, ya que por su hermana su riesgo es de Familiar 1 grado RR 1.8)

Tras esto al clasificarse la lesión encontrada como BIRADS 4 (Breast Imaging Reporting and Data System), significa que es lesión que van a requerir intervencionismo, si bien tienen un rango de probabilidad de malignidad muy amplio (2-95%), por lo que se debe realizar una biopsia

Así se descubre Ca. Lobulillar infiltrante como resultado2

Al conocer el tipo de lesión debe realizarse una RM para determinar la estadificación del cáncer de mama y se encuentran dos lesiones, una de ellas BIRADS 53

Por último hay que saber si el cáncer se ha extendido, por lo que se realiza TAC, GGO y biopsia Ganglio centinela, que nos indican que no existe metástasis3

Se decide como actuación dar quimioterapia neoadyuvante para ver si el tumor es sensible a ella y disminuir el tamaño de la lesión para poder hacer cirugía conservadora, ya que si no, se debe hacer Mastectomía simple o radical más traumática para la paciente4

Por último se da tratamiento Hormonal 5 años con Anastrozol (inhibidor de la aromatasa) ya que ha demostrado beneficio porque disminuye el riesgo de recaídas y aumenta la supervivencia. Otras opciones son análogos LHRH, análogos receptores estrógenos, progestágenos, ooforectomía

También se puede realizar radioterapia postoperatoria, ya que reduce a la tercera parte el riesgo de una recaída local con Braquiterapia de alta tasa perioperatoria, Radioterapia externa con técnica "forward" o ambas juntas
